Once I select a provider, am I assigned to that provider? How often can I change providers?

No, you can select any provider from the network, and you can change providers at any time. Household members can select their own providers. Locate participating providers through our online provider search or by calling Member Services at (800) 290-0523.

How do I verify that a provider accepts the plans?

When scheduling an appointment, please refer to the membership card for proper network identification. Providers will recognize the name of the network with which they participate.

What do I do if I feel the provider overcharged me?

Send a copy of your bill and a written letter to the administrator of the plan, Careington International Corporation, Attn: Provider Relations, P.O. Box 2568, Frisco, TX 75034. They will investigate your inquiry and contact you regarding the findings.

Can I use my membership when I travel away from home?

Yes, your membership can be used at any participating provider in the United States. Locate participating providers through our Provider Search

How do I get additional membership cards?

If you have lost your card or need additional cards for your household, please call Member Services at (800) 290-0523. Select option 1 and then select option 1 again. A Member Services Associate will be happy to send you additional cards.

How do I join?

Enrolling is easy! Choose any one of the following methods:

  2. Call 1 (866) 213-5387 Monday through Friday between 9:00AM and 6:00PM EST
  3. Email info@connectionbenefitgroup.com and a Dental Benefit Club representative will
  4. contact you to collect your contact and payment information.

Everyone is accepted; there are no administrative forms to file; and you can cancel within 30 days and receive a full refund.

How do the plans work?

It is easy to use your discount plan. There are no waiting periods and no limits on use. Once you have joined the plan and received your membership kit, you are free to start using your membership card.

Locate and verify participating providers in your area through our online provider search or by calling Member Services at (800) 290-0523. When scheduling an appointment, please refer to the membership card for proper network identification.

Present your membership card at the time of service to receive a discount. You are responsible for paying the provider the entire discounted fee at the time service is provided.

If my doctor or dentist refers me to a specialist who is not in the network, do I still get a discount?

You will only receive a discount by going to a participating provider. Our network includes specialists in Orthodontics, Endodontics, Periodontics, and Prosthodontics. To receive discounts on procedures in these specialties, share a list of participating providers with your dentist and ask for a referral to one of the providers listed.

Are dental discount plans insurance plan?

When will I receive my membership kit?

You will receive your membership kit 7 to 10 business days after your payment has been processed.
